Using fetch, you'd turn off the "Automatic" button, and choose the "Binary" button. 2) If the file's extension is anything besides ".mid", you have to decode it. ".hqx" is for Binhex, ".sit" is for Stuffit, ".cpt" is for Compactor, ".zip" is for UnZip, ".Z" is for MacCompress (I think), and I'm forgetting the one for UULite. 3) Once you have your ".mid" file, you need to change the type and creator of the file so that your sequencer recognizes it. You can use either ResEdit or FileTyper to do this. The type should be "Midi" and the creator should be "mdos" (quotes not included).
